Role of Clinical Pharmacist in Drug Therapy Management in a Teritary Care Hospital

Abstract: Objectives: Drug Therapy Management is a collaborative approach between physician and pharmacist. Implementation of DTM in hospitals have higher chance reducing DRP’s increase patients’ Quality Life. The aim this study to evaluate types, frequency, ways for minimizing Related Problems identify the role clinical pharmacist Management. Methods: A Prospective Observational was carried out tertiary care teaching hospital Bhimavaram. All in-patients who satisfied inclusion criteria were enrolled. Patient demographics necessary data collected. MS Excel Graphpad Prism 9 Software used analyze data. Chi-Square Test implemented obtain significance number diseases, age, drugs prescribed, length stays with DRP’s. Results: Among 493 cases, total 315 detected (mean = 2.58 per patient). highest occurring DRP Interactions (40.6%). Antibiotics (27.54%) found be highly contributing. major cause inappropriate combination or food (39.75%). discontinuation (26.35%) most appropriate type recommendation. Conclusion: Findings signify need adaptation thereby improving healthcare system. Most medication errors can prevented interventions as they broad knowledge pharmacokinetics, drug use safety.
